Abstract

The frequent involvement of chromosome 17p abnormalities during the evolution of the preleukaemic myelodysplastic syndrome (MDSI and in the transformation of Chronic Myeloid Leukaemia (CML) from the chronic phase to blast crisis led us to investigate the possible involvement of the tumour suppressor gene u53 located on chromosome 17~13. Mutations of the gene are the most common neoplastic change in a variety of tumours and its role as a transcriptional activator of numerous cell cycle genes made it an ideal candidate as a contributing factor in the multistep process of leukaemogenesis. MDS patients previously examined for RAs and FMS oncogene lesions were investigated using polymerase chain reaction (PCR) and single stranded conformation polymorphism analysis (SSCP) on exons 5, 7 and 8 encompassing most of the conserved regions of the gene. Of the 71 patients investigated 3 were found to have ~ 5 3ge ne mutations, 2 also had &&?& mutations. Samples from 19 blast crisis patients were similarly investigated. Six of these patients, 3 of them having polymorphic bands in this initial screen, were sequentially analysed at various stages through their disease progression for 1353 mutations. Further mutations were found in 2 of these patients just prior to transformation from chronic phase to blast crisis. Each polymorphic band was not seen in the subsequent sample following treatment. Abnormalities in u53 do not appear to be common in MDS whereas in CML not only do such abnormalities appear in approximately 20% of the blast crisis samples but may also play a role as a switch factor from chronic to blast crisis phase of disease.
