Abstract

Background: COPD is a multi-system disorder with important systemic co-morbidities and associated consequences. These include increased cardiovascular (CV) risk, loss of skeletal muscle mass and function, osteoporosis and diabetes mellitus. Increased arterial stiffness has been identified in COPD, however there is no validated prognostic CV risk predictor in COPD, as studies so far have only been cross-sectional. Methods: The ARCADE is a longitudinal study which will evaluate the progression and outcome of systemic components of COPD over the medium to longer term. Assessments include pulmonary function (spirometry), arterial stiffness (aortic pulse wave velocity (PWV), blood pressure (BP), body composition and blood biochemistry, amongst others. These will be repeated after two and five years. Results: Thus far, 166 patients have been evaluated, and compared to 77 historical controls similar in age, gender and BMI (Table 1). Patients had greater PWV, BP and heart rate than controls, despite, a similar frequency of previously diagnosed hypertension, diabetes and hypercholesterolemia among the groups. In addition more patients had been diagnosed with osteoporosis, than controls (p < 0.05). Conclusions: The results to date confirm previous findings of elevated PWV and other co-morbidities in COPD. The longitudinal assessments will inform the understanding of the rate and cause of arterial stiffness and other systemic components which may identify links between co-morbidities, and indicate important areas for therapeutic interventions.
